As for who has it, Heatstroke and Stypica on the AZ board have their full setup. You'll have to head to the AZ board to really get a response. I don't see them around here much. I would do a search first either here or there since I know the same ? has been asked a few times.
As for ordering info, I'm sure All-Pro has an 800 number.

I've had mine for about 4 months now and I've been happy with them. Ride well, flex well, haven't starting sagging yet. I throw a fair amount of abuse at them and I have been happy with them so far.

I have the 3" leaf springs as well
they are very stout and I am impressed with the ride

With the springs and 1.5" shackle I got a little over 4".
