white vapor from exhaust on start-up

i have noticed that when i start my truck i have to give it gas and hold it at 1500 rpms for about 30 seconds or the truck will die. it is like when the weather is cold the high idle to help it warm up faster doesnt work anymore. i have also noticed a white vapor coming from the exhaust. does anybody know what could be wrong.

i replaced the spark plugs and cleaned the throttle body and maf 300 miles ago. when the motor gets warm the smoke goes away. what would cause the head gasket to blow? i am going to change the oil this weekend.
